The Liquid Nose Job Guru

In this feature, Amish talks through a recent non-surgical rhinoplasty client case study.

It is not uncommon for us to undertake corrective filler work, but occasionally we have a rare opportunity to improve surgical results. This was the case with a young female client who was now 29 years old and living with a post-surgical nose job that she was desperately unhappy with.

The History

At age 19, the client opted for a surgical rhinoplasty. The client explained that during the pre-op consultation, the surgeon agreed he would break the nose to realign, and shave away the bone of dorsal hump. During the procedure, the client reported to me that the surgeon changed mind regarding breaking the nose and only shaved away bone of dorsal hump.

Following surgery, the client unfortunately developed scar tissue in the area of the dorsal hump, which gave the appearance of dorsal hump returning. This was operated on once, but again, scar tissue returned. The client was very self-conscious about her nose and had been from a very young age.  As well as the bump, she felt that the tip of her nose also drooped heavily and wanted to see if we could improve both.

Facial Assessment

Viewing from the front, the client’s nose kinked to her left side from below the dorsal hump with a noticeable drooping to the tip of the nose. This was significantly exacerbated when the client smiled. From the side profile, I could see the obvious drooping to the tip and appearance of a dorsal hump.

On physical examination, the scar tissue felt very solid, like bone. However, our medical questionnaire revealed that the client had no other history of scaring issues such as keloid scarring.

Creating A Treatment Plan

It was essential to thoroughly educate the client about dermal fillers and the longevity of the treatment. Having never had fillers, it was crucial and clinic protocol to discuss realistic outcomes and risks associated with this procedure. I advised the client that we would achieve a lovely result of the side profile by filling above the dorsal hump and injecting into the top. I would also inject into the base of the nose onto the depressor septi nasi to help lift the tip. I advised the client of the limitations with dermal filler: highlighting that the appearance of the crooked nose from the front could not be fixed with dermal filler. After our initial consultation, the client then booked in a treatment appointment.

The Treatment

On the treatment day, we recapped what we had discussed in our initial consultation, and the client was happy to go ahead. I injected a total of 0.35ml. 0.15 ml in 2 injections sites above the dorsal hump, 0.1ml into tip and 0.1ml onto the depressor septi nasi. For every injection, I aspirated for 10 seconds before slowly injecting a bolus. The whole procedure took approximately 10 minutes.

The Results

The client was immediately happy with the results presented in the mirror directly after treatment with the nose appearing straighter, the tip of the nose lifted and no longer drooping when smiling. We covered treatment aftercare protocol with the client, including highlighting red flags. The client was booked in to return for a follow-up consultation in 2 x weeks.

The client returned for our follow up appointment and was thrilled with the results, citing a feeling of enhanced confidence and happiness as a direct result to this treatment.

Seeing a positive change in a client’s demeanour and confidence post treatment always brings me a sense of great satisfaction. To know that I have made a difference is extremely rewarding in my job and in particular with this client, offering her a long term treatment solution that will continue to deliver the result she loves.
